General Guidelines

Emergency Assistance is available for:
.
• Past due rent (after written Late Notice from the landlord).
We do not assist with boarding/rooming houses/tents or substance
abuse recovery facilities.
.
• Past due mortgage payments (after written Late Notice from
the mortgage company).
.
• Past due utility bills (after Late Notice for gas, electric,
propane, oil, and water). We do not assist with electric or
gas deposits
.
• Deposit monies for rent, first month’s rent, or water.
.
Applicants must have the following documentation for any
type of assistance:
.
• A referral or a letter of introduction from a human
service agency. We honor referrals up to 5 business days (Holidays
and weekends are not included).
.
• A picture identification card (driver's license, state
ID, passport, or a prison I.D.). We accept birth certificates
as a form of I.D. along with a social security card.
.
• A Social Security card, a printout from the Social Security
office,
or a printout from a human service agency worker containing
social security numbers for all members of the household
will serve
as verification.
.
Applicants for food assistance must also have
the following:
.
• Transportation to take groceries home. A one-time overnight
order will be given to those without transportation. With a
referral from a human service agency we will deliver food to
disabled or elderly without transportation.
.
• A referral from a human service agency.
.
• Written documentation that you applied for food stamps, or working with a Human Service agency worker or pastor.
.
• A picture identification card (driver license, state
ID,
or a prison I.D.)
.
• A Social Security card, a printout from the Social Security
office, or a printout from a human service agency worker containing
social security numbers for all members of the household
.
Applicants for financial assistance must also have
the following:
.
• Proof of household income for the past three months.
Please provide your income verification of Work First,
Social Security benefits, unemployment benefits, child support,
etc. If working, we need to see pay stubs or
a printout from
your employer.
.
• Receipts for all household expenses for the past three
months. Examples are rent, utilities, daycare, telephone, car repair,
car payment, furniture or appliance rental, life insurance,
credit card, hospital stay/treatment or doctor visit, back
taxes, legal expenses, etc.
.
• Proof of crisis. An example of a crisis would be that due to an
illness the doctor has taken the applicant out of work causing
them to fall behind in paying the rent or utility bill. We will need
verification from your physician along with expense and income
verification for the past 3 months.
.
• A signed Financial Aid Intake Form that
determines
eligibility.
.
.
• A copy of the bill (in the head household’s name,
not a
child’s name).
.
Applicants for utility assistance must also have
the following:
.
• Disconnection or final notice for their current utility
account or a letter from the utility company to verify the
amount due. They must have a current statement from the utility company of the exact
amount owed.
.
Applicants for rental or mortgage assistance must also have
the following:
.
• Written late notice from the landlord, the magistrate
summons from the sheriff, written past due notice from the
mortgage company, or verification of the amount needed for a rental
deposit or the first month’s rent. They must have a
typed statement from the landlord of the exact amount owed.
This information will also be verified
via telephone.
.
Applicants cannot receive financial assistance if they:
.
• Applied for a service that the Emergency Assistance
Program does not provide.
.
• Have used financial resources which could have
been used to meet their need.
.
• Caused a crisis situation that was self-inflicted or
self imposed that produced the inability to pay the bill.
.
• Have received financial assistance within a year’s
period of time.
.
• Owe on an EAP loan.
.
• Permanently banned due to inappropriate behavior.
.
• Have met the Emergency Assistance Program’s lifetime
limit for financial assistance of $1000.00 (including loans).
Consumers will be eligible for food and clothing and other non-financial
services offered by the Emergency Assistance Program.
